Summary

Patients with HFE p.Cys282Tyr (C282Y/C282Y) homozygous genotype and biochemical evidence of iron overload should be treated with phlebotomy, with the goal of reducing serum ferritin to 50 lg/L. Treatment should be individualized, considering factors such as ferritin level, age, gender, and co-morbidities. Phlebotomy should be performed weekly or every 2 weeks, with a volume of 400-500 mL, and patients should be monitored regularly for hemoglobin and serum ferritin levels. Maintenance phlebotomy should be performed every 1-4 months, depending on the patient's iron status, with the aim of maintaining ferritin levels around 50 lg/L. A healthy, varied diet should be eaten, avoiding iron-fortified foods and high alcohol consumption. Iron chelation therapy may be considered in rare cases where phlebotomy is contraindicated or ineffective.
