The patient, a 33-year-old man with Crohn's disease, developed peripheral neuropathy after taking metronidazole 800 mg three times daily for five days, and then 400 mg three times daily. Electrophysiological investigations and sural nerve biopsy confirmed a distal, mainly sensory, axonal neuropathy. The symptoms disappeared seven months after metronidazole was stopped. Animal experiments using Wistar white rats showed no degenerative changes in the central or peripheral nervous system, but significant amounts of radioactivity derived from metronidazole were bound to RNA. The results suggest that metronidazole or a metabolic product may bind to RNA, inhibiting neuronal function, but the exact mechanism of neurotoxicity is unclear.
