Summary

This case report describes an elderly man with Alzheimer's dementia who presented with agitation, aggression, and delusional behavior. Despite a lack of prior thyroid history, his admission thyroid stimulating hormone (TSH) level was abnormally low at 0.235 mlU/ml. Other thyroid tests were negative. The authors suspected that the TSH abnormality was linked to his acute psychosis.

Methods:

- Serial measurements of TSH and clinical global impression scale assessments were conducted throughout his hospitalization.
- Treatment progress was monitored using the CGI-I scale at admission, after 5 days, and at discharge.

Results:

- The patient's TSH level increased to 0.510 mIU/ml at discharge, corresponding with improved clinical global impression scores.
- Thyroid workup and follow-up tests in 2 weeks were recommended before considering treatment options for thyroid function abnormalities in acute psychotic patients, especially the elderly.
