Summary

A 38-year-old Caucasian man with hyperthyroidism presented with transient weakness and sensory loss in the extremities, which worsened to severe loss of strength, requiring intensive care unit admission. He had started treatment with methimazole and propranolol a week prior. Laboratory findings showed hypokalaemia, mild hypophosphataemia, and elevated liver function tests. The patient was diagnosed with thyrotoxic hypokalaemic periodic paralysis (THPP) and treated with potassium chloride, resulting in complete recovery of strength within one hour. The serum potassium level normalized, and the patient was discharged without physical symptoms. THPP is a rare disease characterized by the triad of hyperthyroidism, hypokalaemia, and brief transient paralysis with proximal muscle weakness, with a higher incidence in Asian men.
