The facility is committed to maintaining the privacy of your health information and has implemented procedures to ensure confidentiality. The Federal Health Insurance Portability & Accountability Act of 2013 requires the facility to maintain the confidentiality of all healthcare records and other identifiable patient health information. You have significant new rights to understand and control how your health information is used. The facility must provide you with this Notice of their legal duties and privacy practices when you first come in for healthcare services. The Notice describes how the facility may use and disclose your protected health information, including for treatment, payment, and healthcare operations. You may restrict disclosures to your insurance carrier for services you wish to pay "out of pocket." You may revoke your consent or authorization at any time, but it will not affect any use or disclosure that occurred prior to revocation.
