A 20-year-old woman with Turner syndrome (TS) was diagnosed with autoimmune polyglandular syndrome type 2 (APS 2), characterized by Hashimoto's thyroiditis and adrenocortical insufficiency, after introduction of recombinant human growth hormone (rhGH) therapy. The patient had been treated with rhGH for short stature associated with TS, and her growth rate increased significantly. However, she developed symptoms of adrenal insufficiency, including hypoglycemia, fatigue, and convulsions, shortly after starting rhGH therapy. Laboratory tests confirmed primary adrenal insufficiency, and the patient was started on hydrocortisone replacement therapy. The patient also had elevated thyroid peroxidase antibodies and anti-thyroglobulin antibodies, indicating Hashimoto's thyroiditis. The case highlights the importance of monitoring patients with TS for autoimmune diseases, particularly when treated with rhGH, and the need for careful assessment of cortisol levels before and during rhGH therapy.
