Summary

Adult COVID-19 patients are categorized into mild, moderate, and severe disease. Mild disease involves upper respiratory tract symptoms and/or fever without shortness of breath or hypoxia, and patients can undergo home isolation and care. Moderate disease is characterized by respiratory rate >30/min, breathlessness, and SpO2 < 90% on room air, requiring admission in a ward. Severe disease involves difficulty in breathing or SpO2 ≤ 93%, high-grade fever/severe cough, and requires admission in HDU/ICU.

Treatment guidelines include:
- Remdesivir for moderate to severe disease with high risk of progression, given for up to 5 days.
- Tocilizumab for rapidly progressing COVID-19, preferably within 24-48 hours of onset of severe disease/ICU admission.
- Dexamethasone 6 mg/day or equivalent dose of methylprednisolone for moderate to severe disease, usually for 5 to 10 days or until discharge.
- Oxygen support, with a target SpO2 of 94-96% (88-92% in patients with COPD).
- Anticoagulation with prophylactic dose of unfractionated heparin or Low Molecular Weight Heparin.
- Monitoring of clinical, lab, and radiological parameters.
- Consideration of non-invasive ventilation (NIV) or high-flow nasal cannula (HFNC) in patients with increasing oxygen requirement.
- Intubation should be prioritized in patients with high work of breathing or if NIV is not tolerated.

Certain medications are not recommended for use in COVID-19, including lopinavir-ritonavir, hydroxychloroquine, ivermectin, neutralizing monoclonal antibody, convalescent plasma, molnupiravir, favipiravir, azithromycin, and doxycycline. Antibiotics should not be used unless there is clinical suspicion of bacterial infection. Systemic corticosteroids are not indicated in mild disease.
