Summary

If patient answered "yes" to Q4 and has been screened before, ask about recent suicidal attempts. If no recent attempts and no other "yes" answers, consider "Low Risk". If only "yes" to Q4, consider "Low Risk" if attempt was over a year ago, patient is in mental health care, parent is aware, and suicidal behavior is not a current concern. Screening criteria include new patients, those not screened in 30 days, positive previous screens, clinical judgement, and 8-9 year olds with behavioral health complaints. Patients 10 and above meeting these criteria should be screened. A positive screen leads to a Brief Suicide Safety Assessment (BSSA) and safety planning, including creating a safety plan, discussing lethal means safe storage, and providing resources.
